Open House, Planning Events
Cottage Health System hospitals in Santa Barbara, Goleta and Santa Ynez will be participating in National Healthcare Decisions Day on April 16 as part of an organized effort to highlight the importance of advance healthcare decision-making. The initiative is aimed at encouraging adults of all ages to plan ahead of a health crisis and put their wishes and decisions in writing to help aid family communication and decision-making as well as enhance patient/provider communication.
Cottage will provide complimentary copies of the Five Wishes Advance Healthcare Directive booklet to help facilitate public and family discussion about written advance directives, power of attorney for health care and communication of individual wishes in regard to end-of-life care.

This document lets your family and doctors know:
- The person you want to make healthcare decisions for you when you can’t make them.
- The kind of medical treatment you want or don’t want.
- How comfortable you want to be.
- How you want people to treat you.
- What you want your loved ones to know.

If you have not yet completed your Advance Healthcare Directive you are invited to visit one
of our facilities and get your copy of Five Wishes. A Cottage representative will be on hand
to answer general information about advance care planning at the one of these locations
and times on April 16:

This open house precedes a day-long workshop on Saturday, April 17 on advance care planning that will be hosted by Santa Barbara Cottage Hospital, in collaboration with Visiting Nurse and Hospice Care, .
